When Should You Start Lymphatic Drainage After Surgery?
Another common question after liposuction, tummy tuck, and other cosmetic procedures is:
“When should I start lymphatic drainage?”
You may hear recommendations ranging from the day after surgery to several weeks later. So what does the research actually say?
There Is No Proven “Best Day”
Despite how frequently manual lymphatic drainage (MLD) is recommended after cosmetic surgery, research has not established one ideal postoperative day to begin treatment.
A 2023 review published in Aesthetic Surgery Journal Open Forum examined the use of lymphatic massage following cosmetic procedures. The authors found evidence suggesting that MLD may help with postoperative edema and recovery, particularly after procedures involving liposuction, but the available studies are limited and treatment protocols vary considerably.
In other words, research supports the potential usefulness of MLD—but it does not currently provide guidelines for when to start.
What To Consider When Deciding When To Start?
The appropriate timing depends on:
The type and extent of surgery
The condition of the incisions
Drain placement
Bruising and tissue sensitivity
Skin and tissue viability
The presence of a seroma, hematoma, infection, or other complication
Your surgeon's postoperative protocol
A patient who underwent limited liposuction may therefore be appropriate for treatment sooner than someone recovering from an extensive tummy tuck, mommy makeover, or another procedure involving significant tissue manipulation.
What Have Studies Found?
A prospective study examined women following abdominoplasty combined with core liposuction. Patients who received MLD for three weeks showed a trend toward less edema at six and eight weeks compared with patients who did not receive MLD.
This studies help support continued investigation of MLD during postoperative recovery, but they do not establish an optimal starting day. Starting on postoperative day one has not been proven superior to starting on day three, five, seven, or later.
The Bottom Line
There is currently no evidence-based universal day when everyone should begin lymphatic drainage after cosmetic surgery.
The decision should be based on the patient's tissues and surgical procedure—not simply the number of days that have passed.
Gentle MLD may be appropriate relatively early after some procedures, particularly liposuction, while other surgeries or individual circumstances may warrant waiting.
The safest approach is individualized care based on the procedure, the condition of the healing tissues, and the surgeon's postoperative instructions.
When in doubt, obtain clearance from your surgeon before beginning manual lymphatic drainage.
