Compression Garments After a Tummy Tuck: What Does the Evidence Actually Show?

Compression garments, fajas, and abdominal binders are commonly recommended after a tummy tuck (abdominoplasty). Patients may be instructed to wear them nearly around the clock for several weeks—or sometimes months—with the idea that compression will reduce swelling, prevent fluid accumulation, support the abdominal repair, and improve the final contour.

But how much of this practice is actually supported by research?

Can They Help Reduce Swelling?

In a randomized study, 32 women undergoing abdominoplasty were assigned to either wear a compression garment or not wear one. Both groups received 10 sessions of manual lymphatic drainage.

Patients who did not wear compression demonstrated significantly less subcutaneous edema later in the postoperative period. By approximately three to five weeks after surgery, waist circumference was lower in the no-compression group. The researchers concluded that compression garments may actually increase rather than decrease postoperative edema in this population.

Can They Help Prevent Seromas?

A 2024 systematic review examined the available research on compression garments and abdominal binders following abdominoplasty. Only five eligible trials involving a total of 130 patients were identified.

The authors found only weak evidence supporting routine postoperative compression, and the available research did not demonstrate convincing evidence that compression prevents seroma formation.

Can Compression Be Too Tight?

The amount of pressure, location of pressure, garment fit, tissue healing, circulation, and lymphatic drainage pathways all matter.

In one of the studies included in the 2024 systematic review, intra-abdominal pressure increased from approximately 2.6 mmHg without a binder to 4.5 mmHg while wearing one.

A 2023 practical review of postoperative compression across plastic surgery similarly concluded that evidence supporting many commonly claimed benefits of compression remains limited. Other concerns discussed in the literature include effects on venous blood flow and respiratory mechanics.

Is Longer Compression Better?

A 2026 randomized controlled trial provides particularly useful information about how long compression may be necessary.

Researchers studied 120 patients undergoing abdominoplasty with rectus diastasis repair. Patients were randomly assigned to wear a standardized Class II abdominal compression garment for:

  • 1 month

  • 3 months

  • 5 months

Researchers then evaluated abdominal wall stability, swelling, venous-lymphatic function, intra-abdominal pressure, strength, return to activity, complications, comfort, and patient satisfaction.

At six months, abdominal wall stability in patients who wore compression for only one month was not inferior to patients who continued compression for three or five months. Longer compression also did not improve final contour, complication rates, or patient satisfaction.

Instead, prolonged compression was associated with slower resolution of edema, reduced venous flow, higher intra-abdominal pressure, reduced comfort, and slower functional recovery during the earlier stages of rehabilitation.

Patients who discontinued compression earlier returned to activity sooner and demonstrated better early trunk strength.

The researchers concluded that extending compression beyond the first postoperative month did not provide additional benefit and suggested approximately one month followed by gradual weaning as a reasonable approach for the population they studied.

Do These Findings Apply To Everyone Getting A Tummy Tuck?

No.

Abdominoplasty is not one standardized procedure. A patient undergoing a straightforward abdominoplasty with rectus plication may have very different postoperative needs from someone undergoing:

  • extensive liposuction

  • 360-degree liposuction

  • a mommy makeover

  • massive-weight-loss body contouring

  • multiple simultaneous procedures

  • revision surgery

  • treatment involving significant tissue undermining

Garment type and pressure also vary considerably.

Most importantly, your operating surgeon's postoperative instructions take precedence. Your surgeon knows exactly what was performed and whether there are specific reasons compression is being recommended.

The research instead tells us that prolonged compression should not automatically be considered better simply because it is longer or tighter.

The Bottom Line

Compression remains a common component of tummy tuck recovery, and it may provide support and comfort during early healing. However, current research does not strongly support the idea that more compression or longer compression automatically produces less swelling, fewer seromas, or a better cosmetic result.

In fact, emerging evidence suggests that prolonged compression may delay edema resolution and temporarily affect venous flow, intra-abdominal pressure, comfort, and functional recovery in some patients.

The goal should therefore not be maximum compression, instead appropriate compression for the individual patient, procedure, and stage of healing.
